fix: regex was matching w:tbl, w:tc, w:tr — corrupting DOCX XML

The previous regex /<w:t(?!...)/ matched any tag starting with <w:t
including <w:tbl>, <w:tc>, <w:tr>. Added lookahead (?=[ >/]) to only
match <w:t> elements (followed by space, > or /).
